There is a place for a current photo (be sure to update yearly), your child's fingerprints and medical history. Pre-inked fingerprint strips will be shipped along with this item. For assembly, see option below.

ASSEMBLY OF KITS
This additional charge is for the placement of a pre-linked fingerprint strip within the folds of each individual ID Kit. This charge also applies to placing any kit listed above into a plastic sleeve.
PLASTIC SLEEVE
A 4" x 6" clear protective sleeve is available for all ID Kits listed above.
